Appointment requests
The form collects your name, mobile number, preferred branch, optional preferred date and consent to contact you. Authorised clinic staff can access requests to arrange a consultation. Please do not enter medical records or sensitive clinical details in the form.
Website services
This website uses Cloudflare for hosting and storage. Appointment enquiries are also copied to a private Google Sheet, and an email notification with the submitted contact details is sent to the clinic’s designated email address for follow-up. Google Maps loads when you view branch maps, and Google Fonts provides the site typefaces. These services receive technical connection information, such as your IP address. The preview does not load advertising trackers.
